You
need to include the ORIGINAL hand written EMS report and the
hard cardstock portion of the run sheet with the patient's
signature on the back with the paperwork you turn in.
Failure to turn in this required documentation may result in you
having to re-write the entire run report.

The
new Incident Report sheets are now out. You will notice a
change to the Personnel column. There are three columns
after the names. Here is how they are to be used:
Station:
If you stayed at the station and were not part of any responding
crew place a check beside your name here.
Truck
#: If you were a part of the Incident Response place the
unit number you responded with in this column.
Day
Crew: If you are getting paid as part of the Day Crew /
Station Crew place a check in this box. You will still
need to mark the
Station or Truck # box as to what part you took in this
particular incident.

All Day Crew, Station Crew, and
"On-Call" sign up sheets for the rest of the year are
at the station. Please sign up as soon as you can.
REMINDER: Sign Up's are removed the 20th of each month to
complete scheduling.
